In state of uttar pradesh, Handicapped certificate is issued
by the health department of Up. All physically disable candidates who are handicapped
from birth or have disability after the birth they can apply for physically handicapped
PH certificate. PWD certificate will be
issued on the basis of percentage of the disability and type of disability. Government
will provide different typed of scholarships, reservation, and other facility ph
candidates on the basis of विकलांग प्रमाण which is issued by Swasthya vibhag. You can also use handicapped
certificate for getting benefit in government scheme, admission in education institutes
, pension scheme or getting government Job and free IAS, PCS Coaching .
Eligibility criteria for PWD Certificate – any personal who
are physically disable can apply for Viklang praman patra.
How to apply: eligible candidates can apply for physical handicapped
certificate in prescribed application form format. Attach all required supporting
document with this application form.
After submission of dully filled application form in
District government hospital, all medical tests will be held by the government
doctor. Cocnered medical team will decide the handicapped type and percentage
of Disability. After that that Swasthya vibhag will issue your Physical handicapped
certificate.

- Affidavit certificate
- 4 colored size photograph
- Cast certificate for getting the benefit in the application form.

Application form fee- rs 32 for general category candidates and rs 8
for sc category candidates.
